Bruising

Nerve damage

Needles under nails may damage extensor tendon if pushed too deep

Difficult to return bottom to normal state quickly in case of safeword use

Needles hitting vans deferens may cause infertility

Needles in penis can collapse or block urethra

Specific to sewn/stapled/needled mouths: what happens if vomit?

Sutures risk ripping skin if tension too tight

Possibility of ink transfer if designs are drawn on and cut/needled over

Pathergy -- Blistering or sores that result as an immune response to punctures of the skin

Those with severe latex allergies are at risk from the small amount of latex used to secure a needle to its hub
